This section outlines the necessary steps and required procedures in order to schedule an exam through the Frost Campus Testing Center in Beracha Hall.
Before initiating an OCT request, the student must have consulted with the instructor, whose permission is required for the student to take the exam at the Testing Center. Whether or not to use the proctoring services of the Frost Campus Testing Center in Beracha Hall, must be discussed with the instructor. If the student and instructor mutually agree to use the Testing Center, the student should proceed to contact the Testing Center to obtain forms and detailed information regarding procedures and scheduling.
Students who desire to take an exam at the Testing Center but who do not have permission of the instructor may either follow the exam recommendations of the instructor or consult with Disability Services regarding exam accommodation concerns.
Because the Testing Center is an optional supportive resource, exams cannot be scheduled earlier or later than the hours of operation. For exams that need to occur earlier or later than 8:00 AM - 4:30 PM, course instructors will need to either proctor the exam within the department or adjust the time of the exam to fit within the hours of operation.Completing the OCT Form
In order to schedule an OCT at the Frost Campus Testing Center in Beracha Hall, an OCT form for each exam must be completed and submitted by the student by the appropriate notification deadlines to the Center. Forms may be completed electronically or via hardcopy. The OCT form must be submitted five (5) business days prior to the exam/test date.
The student must complete the upper portion of the form indicating:
· Name and email address of student;
· Course name and number;
· Name and email of instructor;
· Day, date, and time for the requested exam; and
· Accommodations requested.
After the student has completed the upper portion of the OCT form, the student must email or hand-deliver the OCT form to the instructor. For the first scheduled OCT exam of the semester, the instructor must complete the bottom portion of the form indicating:
· The actual date and time of the in-class exam;
· The length in minutes of the in-class exam;
· Any special instructions or additional materials allowed during the exam;
· Method of delivery of the exam to and from the Testing Center; and
· Whether or not future Out-of-Class-Testing requires that the instructor completes the OCT form.
As a courtesy to the professor, the student should plan for the instructor to take at least one business day to complete the form.
While the student portion of the OCT form must be submitted by the student each and every time an OCT is to be scheduled at the Testing Center, the instructor can choose either to complete the OCT form only for the first OCT scheduled in the semester or every time an OCT is to be scheduled. The instructor should indicate future preference on the OCT form when it is completed initially. However, whenever the exam format or requirements, such as additional equipment, supplies, or time allowances change over the course of the semester (except for Final exam time allowances), the instructor must complete the information on the OCT form in order to provide the Testing Center staff with the most accurate information.
Although communication between student and course instructor for scheduling an OCT exam can be completed by email, the responsibility for timely submission of the completed OCT form is always that of the student, not the instructor. Failure to submit the completed OCT by the appropriate deadline will result in issuance of warnings and/or penalty, including the possibility of denial of the service requested.
The OCT form may be emailed to email@example.com.
Confirmation of the scheduled OCT will be emailed to students and to course instructors generally within 2 business days of receipt of the completed OCT form. After submitting the form, the student should allow at least one business day to elapse before contacting the Center to verify receipt and/or processing of the OCT form.
On the date of the scheduled OCT, the student should report at the scheduled time to the Frost Campus Testing Center in Beracha Hall Room 108. To check-in, the student must present the student ID card to the Testing Center staff. The student will be issued a key for a locker in which to place belongings.
If a student arrives more than 15 minutes after the OCT scheduled time, the Testing Center staff will notify the instructor. Adjustments to the start time and possible effect on overall time accommodation will be determined by the Testing Center staff in consultation with the course instructor. Depending on the circumstances at the time of the scheduled OCT, students arriving late to a scheduled OCT may forfeit some of their time accommodation or may lose the opportunity to take the exam as scheduled.
Students are expected to behave in accordance with the highest standards of academic integrity and to follow all posted Testing Center Guidelines. Students may have access only to materials explicitly stated as approved by the instructor on the corresponding completed OCT form.
Should a student be caught using materials not approved for the exam, the Testing Center staff will abruptly stop the exam, collect or record the materials used, and will return the exam and other materials to the course instructor. The student will be referred to the course instructor, who will be responsible for handling the matter in accordance with course and academic unit policies.
In the event a question arises before or during a test, the Testing Center staff will attempt to contact instructors by phone or email. When immediate contact is not possible, students will be given the opportunity to write questions or concerns on a note to be returned along with the completed exam to the instructor. Instructors who become aware of any errors, omissions, or necessary clarifications while students are taking an exam should email firstname.lastname@example.org or call 314-977-5176 as soon as possible.
Assuming timely arrival at the Testing Center, the student will be provided with the full time accommodation stated on the corresponding OCT form. Once the accommodated time has elapsed the student must surrender the exams and appropriate materials to the Testing Center staff.
Once the student has completed the OCT, the Testing Center staff will collect the exam and appropriate materials to be returned to the instructor.
Unless otherwise requested by the instructor, completed exams are delivered daily, Monday thru Friday, from 2:00 PM to 4:00 PM to the department mailbox of the instructor. Department mailbox information is ascertained through People Finder. Should an instructor require a completed exam prior to the 2:00 PM thru 4:00 PM delivery time, the instructor will need to personally retrieve the exam during the Testing Center regular hours of operation. Exams that are completed after the 2:00 PM delivery start time will be returned on the following business day between the hours of 2:00 PM and 4:00 PM.
