Please Note on the Interactive PDFs: When you click the link, go to File>>Save As, give the document a new name, then Save the newly named file on your computer. Open the PDF file in Acrobat Reader, fill out the fields on the forms, then again do a File>>Save As and rename your completed file. Then print your completed file and send through campus mail for the proper authorizations and signatures. When that is completed, send the approved document Dr. Reinhard Andress, Associate Dean for Academic Affairs, The Graduate School at Verhaegen Hall 106.

Commonly Used Forms

• Application Recommendation from the Proposed Major Field
  Used only by authorized Admissions Coordinators, Admissions Directors, and Department Chairpersons
• Course Approval Form (NEW Interactive PDF)
  Used to propose a new 500- or 600-level course to be added to the permanent offerings of the specific department or program. A different form is to be used to propose a special-topics course.
• Special Topics Approval Form (NEW Interactive PDF)
  Used to propose a special-topics course to be offered by a visiting faculty member or on a trial basis.
• Notification of Readiness for the Public Oral Presentation of the Ph.D. Dissertation
  Must be completed and sent to the Doctoral Candidacy Advisor at least three weeks before the date of the public, oral presentation of a dissertation.
