Home visits from SLUCare physicians
Do you or does someone you know have:
- Trouble getting to doctor office appointments due to impaired mobility or transportation problems?
- A terminal illness?
- Advanced bedsores or other multiple wounds?
- Alzheimer's dementia or advanced Parkinson's disease?
- Frequent falls?
- Tube feeding?
- Urinary incontinence?
If the answer to any of these questions is "yes," you or your loved one may qualify for SLUCare's home care program.
Our physicians typically see home visit patients once every three months, but may visit more or less often, depending your needs. Our physicians communicate with your primary care physician by letter or telephone, or we can serve as your primary care physician.
Many health insurance plans have some restrictions about physician home visits. We would be more than happy to discuss this with you.
