New Dependents: New dependents acquired through marriage, adoption or placement for adoption must be enrolled within 30 days of the date of marriage, adoption or placement for adoption. Required Documentation: Enrollment form. Additional documentation substantiating qualifying event date may also be required.
Newborns: An enrollment form must be submitted within 30 days of the child's date of birth. Any applicable premium will be calculated from the child's date of birth. Required Documentation: Enrollment form. Additional documentation of birthdate may also be required.
NOTE: Social Security Numbers are NOT required to complete a newborn dependent's enrollment form. Members are advised to submit newborn enrollment forms within 30 days even if a Social Security Number has not been obtained. Social Security Numbers can be provided later once issued.
NOTE: Upon birth, Aetna may route temporary identification cards that list newborn dependents. These temporary cards are provided to reflect 30 days of mandated newborn coverage. Regardless of receipt of temporary identification cards, a formal enrollment application adding a newborn must be submitted to the plan within 30 days of birth if coverage is desired beyond the 30 days of mandated newborn coverage.
