Q: How do I make sure my home address is correct for W-2 statement mailings?
A: If your home address has changed during the last 12 months, please notify your department or call the human resources information office at 977-2305 for instructions.

Q: I have a serious health condition and will need to be off work for four to six weeks. What should I do to request leave under the Family and Medical Leave Act (FMLA)?
A: The FMLA provides eligible employees with up to 12 work weeks of job-protected leave per 12-month period for qualifying reasons. In the case of a serious health condition, first you must obtain an FMLA request form from either your supervisor or the human resources department. Once you have filled out this form, you should turn it in to your supervisor for review. The supervisor then will send you a response form letting you know whether your FMLA request has been granted. At this time, you may be required to provide medical certification from your physician. If your leave is granted, you should then set up a payment plan with your supervisor (all accrued vacation and sick leave must be used before you can take unpaid FMLA leave), and arrange for start and end dates for your leave (you may take a maximum of 12 weeks of FMLA leave per calendar year). Shortly before the end of your leave, call your supervisor to let him or her know that you will be returning to work. Please note that designating leave as FMLA is not "optional"; if a leave is for an FMLA qualifiying reason, the employer is responsible for ensuring that the leave time is indeed designated as FMLA leave.
