New Employee Information

New Faculty Employees

Physicals
Health Sciences Center faculty members working in clinical or laboratory areas are required to complete a pre-employment physical. Physicals are required by the University's Employee Health Department BEFORE the first day of employment. Physicals for new faculty members may be scheduled by calling Employee Health at (314) 268-5499 Ext Zero. (Please keep in mind that sometimes there is a waiting time before an appointment can be obtained, so it is best to call early). Additionally, Tuberculosis Screening must be complete prior to clearance. The PPD reading cannot be performed until 48-72 hours after administration. (See attached for information, including required documentation.) Frost Campus faculty members are not required to complete physicals.

Benefits
Frost Campus and Health Sciences Center new faculty members have 31 days in which to enroll in a medical benefits plan, and to submit other benefit forms. Benefit orientations are held weekly. Benefit packets are distributed during this orientation and a Benefit Specialist is present to assist faculty in completing forms and to answer benefit questions. To obtain information regarding new faculty benefit orientation dates, faculty members should contact the Benefits office on or before their first day, at (314) 977-2304.

Employment Eligibility Verification (I-9) and Tax Information
Frost Campus and Health Sciences Center new faculty members are required to report to the Human Resources office to complete employment eligibility and tax forms on or before their first day of employment. The law requires that Section 1 of the I-9 be completed at the time of the hire-when the employee begins work and that acceptable documents listed on the back of the I-9 be presented to the Human Resources department within three (3) business days of the hire date. New faculty members are required to bring original documents to establish identity and employment eligibility. (Attached is a list of acceptable documents to establish employment eligibility.)

Identification Cards and Parking Passes
Faculty/Staff Temporary identification cards (IDs) are issued in the Human Resources office on or before the first day of employment. The Personnel Action Form (PAF) and Employment Requisition Form (ERF) must have previously been submitted to Human Resources by the department and the above documents (I-9, tax forms) completed by the faculty member, and a physical is required to have been completed before an ID is issued.

Frost Campus faculty members may obtain their Faculty Identification and Parking Pass at either of the two locations of Parking and Card Services. The office located on the North end of campus is in DuBourg Hall, room 33, 221 N. Grand. Business hours are Monday through Friday from 8:30-5:00 and on Wednesday they are open until 5:30 p.m. Present the Temporary ID Card that was obtained in Human Resources Department.

Health Sciences Center faculty members may obtain their Faculty Identification and Parking Pass at either of the two locations of Parking and Card Services. The office located on the South end of campus is at 1310 South Grand, in the Macdonald Building. Business hours are from 7:30-11:30 and from 12:00-4:00 Monday through Friday. Present the Temporary ID Card that was obtained in the Human Resources Department.

Criminal Background Record Checks
Health Sciences Center. Recent legislation requires that criminal record checks be performed for all prospective faculty hired into positions that will have patient contact (direct or indirect). Criminal Background Record Check request forms are completed and witnessed in Human Resources. Ideally, this form should be completed prior to the first day of employment, as it takes 48 hours to obtain the results.

Frost Campus faculty members who do not have patient contact are not required to have a criminal background record check performed.

Faculty Orientations
Frost and Health Sciences Center faculty orientations are coordinated through the Office of the Provost and the respective Deans office. New faculty will receive information regarding upcoming faculty orientations from their respective School or College.

University Medical Group Compliance (SLUCare)
Health Sciences Center. All new University Medical Group (UMG) faculty members are required to contact the Compliance Department to schedule a compliance educational session. This session is required for those UMG Faculty members who will be billing for their services. The contact with the Compliance Department should be made within the faculty member's first month of employment by calling (314) 268-5545.
Frost Campus. UMG Compliance is not applicable for Frost Campus faculty members.
