Saint
Louis University offers comprehensive benefit plans, including
medical, life insurance, long-term disability income protection,
dental, accidental death and dismemberment, retirement, dependent
care and tax-deferred annuities/mutual fund options. All benefit
plan options can be viewed on the Internet at www.slu.edu/services/HR/benefits_home.html.
This
handbook includes only a general outline of the benefits available.
Information concerning each plan may be found in the appropriate
summary plan description available through the University
benefits office or on the benefits Web site listed above.
All conditions of coverage are governed by a plan document.
The plan document for each benefit plan is maintained in the
university benefits office. The University reserves the right
to change the structure of the benefit plan, including eligibility,
at any time.
New
employees are given 31 days from their date of full-time employment
to enroll in the medical, dental, flexible spending, dependent
care, life insurance, long-term care, and accidental death
and dismemberment plans. It is the employee's responsibility
to attend a benefits orientation in order to receive benefits
information.
Benefit
Eligibility
Full-time
staff members are eligible for all of the plans described.
"Full-time" is defined as the assignment to a full-time
budgeted position scheduled at least 64 hours per pay period
or .8 FTE on a regular continuing basis. Temporary staff members
are not eligible. Retirement plan benefit eligibility requires
one year of service based upon a minimum of 1,000 hours.
Tax-deferred
annuities and tax-deferred mutual funds are available to all
staff members.
Accidental
Death and Dismemberment (AD&D)
The
University provides accidental death and dismemberment benefits
for full-time staff members. Additional benefits are available
through payroll deduction. Family benefits are also available
for a spouse and/or dependent children. An employee may make
updates to his or her AD&D plan at any time.
Adoption
Assistance
The
University provides assistance to eligible staff who are building
their family through adoption in the form of an adoption expense
reimbursement plan. Staff members who have completed 12 months
of employment and have met the eligibility requirements for
the Family and Medical Leave Act policy (please see Section
G) are eligible for this benefit.
Eligible
adoption-related expenses will be reimbursed to a maximum
of $6,000 for agency and non-stepchild adoptions. For adoption
of a stepchild, the adoption reimbursement maximum amount
is $1,000.
Dental
Plan
Saint
Louis University offers a pre-tax, voluntary dental plan for
full-time staff members and their dependents. There are plan
choices available, including PPO in-network and/or out-of-network
coverage. After a new employee's initial enrollment period,
enrolling, changing, or terminating coverage is only allowed
during the annual open enrollment period.
Dependent
Care Plan
The
Dependent Care Plan lets staff members pay for eligible child
or dependent care expenses with contributions taken from their
pay before taxes are withheld. Reductions may be as high as
$5,000 per year depending on participation. Married persons
filing separate income tax returns would be limited to $2,500.
Enrollment in the plan is for a calendar year. Participants
must re-enroll annually during the open enrollment.
Employer
Assisted Housing
In
an effort to contribute to the revitalization of the immediate
University neighborhood, and assist its employees in the pursuit
of home ownership, Saint Louis University seeks to provide
a housing benefit to its employees through the Hometown SLU
program. The goal of the Hometown SLU is to provide three
benefits for University employees: home ownership education,
special lender offers, and a forgivable loan program.
Flexible
Spending Plan
The
flexible spending plan allows employees to pay for eligible
medical and dental expenses that are not otherwise covered
by insurance with contributions taken from their pay before
taxes. Staff members may elect an amount of benefit up to
$5,000 per year. Enrollment in the plan is for a calendar
year. Participants must re-enroll annually during the open
enrollment.
Life
Insurance Benefits
The
University provides basic life insurance for full-time staff
members. Additional life insurance is available through payroll
deduction. Dependent life insurance is also available. An
employee's additional life insurance benefit may be reduced
at any time; however, to apply for an increase in benefits
an employee must complete evidence of insurability forms.
In addition, a "living benefit" option allows an
employee who is terminally ill to obtain cash from the basic
life insurance benefit to use for final expenses.
Long-Term
Care Insurance
The
University offers enrollment in a Long-Term Care Insurance
program. Additionally, eligible spouses, adult children, spouses
of adult children, siblings, spouses of siblings, parents,
parents-in-law, grandparents, grandparents-in-law of eligible
faculty, staff and housestaff also may enroll, under certain
conditions. This program is a voluntary benefit and participants
are billed at home for this plan. New employees enrolling
within 31 days of employment have guaranteed acceptance regardless
of their health status. Subsequent enrollment is subject to
underwriting.
Long-Term
Disability
After
one year of continuous employment, Saint Louis University
provides long-term disability insurance designed to replace
a portion of lost income in the event of disability.
The
University pays all of the cost of the benefit coverage up
to $36,000 of annual salary for full-time staff members. There
is a payroll deduction charge on amounts of salary above $36,000.
Participation in the plan is mandatory as a condition of employment.
Medical
Insurance Program
Saint
Louis University offers several different medical plan options.
The University pays a significant portion of the cost of medical
benefits; however, there is a pre-tax payroll deduction for
all participants. The benefits of the medical plans are described
in the summary plan description available from the University
benefits office. After a new employee's initial enrollment
period, enrolling, changing, or termination of coverage is
only allowed during the annual open enrollment period, unless
a qualifying event occurs. Contact the benefits office for
details.
Retirement
Plan
The
University, after one year of service, will provide a contribution
determined by the contribution election of the employee. Refer
to the summary plan description available from the University
benefits office for details.
403(b)
Tax-Deferred Annuities/Mutual Funds
Voluntary
contributions may be made on a tax-deferred basis to a variety
of investments, including tax-deferred annuities and mutual
funds. There is no waiting period for participation. The tax
advantages of long-term savings through this program can be
significant. Contact the University benefits office for details.
Tuition
Remission for Staff
Tuition
remission for undergraduate and graduate courses taken at
Saint Louis University is available for all full-time staff
members. Eligible staff members must apply for this benefit
online and are responsible for a handling fee per credit hour.
The handling fee must be paid to the bursar's office upon
the start of each course. Application for tuition remission
is available online through Banner Self Service at https://selfserv.slu.edu
Tuition
Remission for Spouse and Children
Tuition
remission for undergraduate courses taken at Saint Louis University
is available to the spouse and children of staff members who
qualify. Application for tuition remission is available online
through Banner Self Service at https://fsselfserv.slu.edu.

The
University participates in the faculty and staff children
exchange program (FACHEX), which is an undergraduate tuition
remission program for children of full-time employees, at
participating Jesuit institutions. It permits a limited number
of children eligible for tuition at the home institution to
receive the same benefit from 25 other Jesuit colleges and
universities. The benefits are for the remission of tuition
only. Students participating in the program are expected to
pay all fees and room and board charges assigned by the institution
receiving the student. Generally, the tuition remission pertains
to the regular sessions only. To learn more about FACHEX,

Tuition
Exchange Program
The
tuition exchange program enables children of eligible Saint
Louis University employees to apply for a tuition exchange
program scholarship at any of the more than 480 tuition exchange
program participating colleges and universities. Applications
for limited slots in the tuition exchange program are due
by December 1st, before the student's fall semester of enrollment
at a participating exchange institution. Please note
