POLICY
It
is
the
policy
of
Saint
Louis
University
to
notify
the
University
community
of
the
death
of
an
employee,
an
employee's
spouse,
child,
or
parent
in
order
for
the
employee
and
his
or
her
family
to
be
remembered
in
the
thoughts
and
prayers
of
colleagues
and
friends.
The
department
head
of
the
employee
should
designate
a
contact
person
within
the
department
who
will
notify
by
phone
the
campus
Human
Resource
Office
of
the
death,
as
well
as
information
regarding
funeral
arrangements,
and
a
family
member
and
address
for
condolences.
The
Public
Relations
Department
should
be
contacted
immediately
regarding
the
death
of
retirees,
emeriti
faculty,
or
other
prominent
individuals.
The
Human
Resource
Office
will
prepare
a
statement
which
will
include
(1)
the
employee's
name,
(2)
the
name
of
the
deceased,
if
not
the
employee,
(3)
University
connection
(i.e.,
department,
job
title,
length
of
employment),
(4)
name
of
family
member
and
address
for
condolences,
(5)
date
of
death,
(6)
wake
time
and
location,
(7)
funeral
time
and
location,
and
(8)
departmental
contact
person
and
telephone
extension.
The
campus
Human
Resources
Office
will
formally
notify
the
following
people:
