form must be each Fall for Full-Time undergraduate students.
You will also be required to submit a valid copy of your insurance
card to be held on record in the office of Health and Counseling.

Health Plan Enrollment Form |

This is
required only if student does not have medical insurance.
If the student does not provide evidence of medical insurance
the student will automatically be enrolled in this program.

you are a freshman who will be residing on campus, you must complete the menigococcal
vaccination requirement either by providing documentation of vaccination or by
completing the waiver section of the form indicating that you have received the
information and have made the decision not to have the vaccination.
