Out-of-Class Test (OCT) Scheduling Procedures
The following information outlines the procedures for scheduling exams through the Testing Center within the Student Success Center Medical Center Campus in 114 School of Nursing building.
In order to schedule an OCT at the Testing Center within the School of Nursing, an OCT form outlining all exams to be taken in the course for the semester must be completed and submitted by the student to email@example.com
The student must complete the upper portion of the form indicating:
· Name and phone number of student;
· Course name and number;
· Name and email of instructor;
· Accommodations requested;
· Exam schedule for the semester for the course in question.
After the student has completed the upper portion of the OCT form, the student must email the OCT form to the instructor. The instructor must complete the bottom portion of the form indicating:
· Verification of listed exam schedule;
· Any special instructions or additional materials allowed during the exam;
· Method of delivery of the exam to and from the Testing Center
As a courtesy to the instructor, the student should plan on the instructor taking at least one business day to complete the form.
Although communication between student and course instructor for scheduling an OCT exam can be completed by email, the responsibility for timely submission of the completed OCT form is always that of the student, not the instructor. The OCT form must be emailed to firstname.lastname@example.org
On the date of the scheduled OCT, the student should go to the Testing Center within the Medical Center Student Success Center at the scheduled exam time. If a student arrives more than 15 minutes after the OCT scheduled time, the Testing Center staff will notify the instructor. Adjustments to the start time and possible effect on overall time accommodation will be determined by the Testing Center staff in consultation with the course instructor. Depending on the circumstances at the time of the scheduled OCT, students arriving late to a scheduled OCT may forfeit some of their time accommodation or may lose the opportunity to take the exam as scheduled.
Students are expected to behave in accordance with the highest standards of academic integrity and to follow all posted Testing Center Guidelines. Students may have access only to materials explicitly stated as approved by the instructor on the corresponding completed OCT form.
Should a student be caught using materials not approved for the exam, the Testing Center staff will immediately stop the exam, collect or record the materials used, and will return the exam and other materials to the course instructor. The student will be referred to the course instructor, who will be responsible for handling the matter in accordance with course and academic unit policies.
In the event a question arises before or during a test, the Testing Center staff will attempt to contact instructors by phone or email. When immediate contact is not possible, students will be given the opportunity to write questions or concerns on a note to be returned along with the completed exam to the instructor. Instructors who become aware of any errors, omissions, or necessary clarifications while students are taking an exam should email email@example.com or call 314-977-8841 as soon as possible.
Assuming timely arrival at the Testing Center, the student will be provided with the full time accommodation stated on the corresponding OCT form. Once the accommodated time has elapsed the student must surrender the exams and appropriate materials to the Testing Center staff.
Once the student has completed the OCT, the Testing Center staff will collect the exam and appropriate materials to be returned to the instructor.
Completed exams are delivered daily at the conclusion of the day's exam schedule. Instructors who desire the completed exams sooner than later are asked to make arrangements to personally pick up the exams.
