Endodontics and Periodontics
Patients interested in receiving endodontic or periodontic care at Dental Specialties of Saint Louis University must be referred by a dentist or other medical professional.
Referring physicians, please call 314-977-8381 or download a patient referral form and return it by fax to 314-977-8383.
Orthodontics
For records requests, download the authorization for disclosure form and return it to our office once completed. A fee may be charged for duplication costs.
Imaging Center
Use the Imaging Center referral form to refer a patient to our Imaging Center for a CBCT scan.
