Appointments Available for 2019 Vitality Checks

SSM Health Saint Louis University Hospital's Outpatient Services will offer Vitality Checks for 2019 to Saint Louis University employees. Vitality Checks are required for those SLU employees and their covered spouses to receive a discount on health insurance in 2019.
Appointments will be conducted at the Simon Recreation Center from 6 a.m. to 12:55 p.m. Thursday, Oct. 1 through Friday, Oct. 26. Appointments will be also conducted at the SSM Health SLU Hospital Outpatient Services clinic at 3655 Vista Ave. from 6 a.m. to 12:55 p.m. Monday, Oct. 29 through Friday, Nov. 16.
Limited Saturday appointments will also be available at the outpatient clinic on Oct. 13, Oct. 27, Nov. 10 and Nov. 17 and on Sunday, November 18. All weekend screenings will be conducted at SSM Health SLU Hospital Outpatient Services clinic.
Registration is available online. Those with special needs are encouraged to call 314-685-3398 rather than enrolling online.
The free screenings are available by appointment only. Walk-in appointments will not be accommodated. To cancel or confirm your appointment, please call 314-685-3398.
Results from the Vitality Check screenings conducted by SSM Health SLU Hospital Outpatient Services clinic may be mailed via interoffice mail to employees or you may logon to MyChart to view your results.
As in the past, employees are also welcome to go to their personal physicians. However, additional charges may apply.
In order to be eligible for the Wellness medical premium discounts for 2019, employees and their covered spouses must complete both the Vitality Check biometric screening as well as the Vitality Health Review (VHR) through www.powerofvitality.com between Monday, Oct. 1 and Friday, Nov. 16.
